The LVN Charge Nurse position at Silsbee Oaks Health Care LLP is a full-time role that requires providing nursing care to residents as prescribed by physicians and in accordance with nursing standards and regulations. The nurse will lead and direct both licensed and non-professional staff in delivering direct resident care and support functions. The position is scheduled for overnight shifts from 10 PM to 6 AM, with a rotation of four days on and two days off, including weekends as needed. A sign-on bonus of $2000 is offered, along with a $2 per hour shift differential. In this role, the LVN Charge Nurse will ensure resident safety and provide the best possible nursing care by planning, organizing, and directing nursing functions for patients in the unit. Responsibilities include making appropriate nursing assignments based on the skill level of employees, maintaining acceptable standards of patient care, identifying problems, and guiding personnel to solutions. The nurse will also be responsible for accurately implementing physician orders, supporting infection-control policies, and maintaining confidentiality while cooperating with nursing staff, medical staff, and visitors. The LVN Charge Nurse will obtain and document comprehensive nursing histories from patients and/or their significant others, perform basic assessments, record vital signs, and notify physicians of any changes in patient conditions. They will develop care plans based on assessment information and ensure that care plans are communicated effectively to incoming personnel. The nurse will also be accountable for the clinical care of assigned patients, including the supervision of non-professional personnel, and will communicate effectively with patients, families, and other healthcare team members. The role requires adherence to established standards, policies, and procedures, and the ability to respond appropriately in urgent situations. Physical requirements include the ability to perform movements such as pushing, pulling, lifting, bending, kneeling, and reaching, with the capacity to lift up to 50 pounds. The work schedule may include evenings and holidays, and the position is classified as non-exempt.
