About The Position

The LVN Case Manager I is responsible for coordinating high-quality nursing support services at the LA Family Housing Facility. This role involves working with program participants in their living units or communal spaces. As an essential member of the healthcare team, the LVN Case Manager I collaborates with physicians, mid-level clinicians, Registered Nurses, the Administrator, and other professionals to manage patient care. This position is funded by a grant from the Department of Mental Health, the Department of Health Services, and the LA Family Housing Corporation.

Requirements

  • Must have graduated from an LVN program. While this is an entry-level position, experience in an ambulatory care facility with a focus on patient education is highly desirable. Experience as a Case Manager is preferred. Experience must have been as a Licensed Vocational or Technical Nurse within the United States Licensing Jurisdiction.
  • Must maintain a current Cardio–Pulmonary Resuscitation certification, in compliance with corporate CPR policy.
  • Thorough working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures.
  • Math skills sufficient to prepare routine spreadsheets, calculate/balance invoices/expense reports, and  similar.
  • Computer skills in Microsoft Office programs (Word, Excel, etc.), electronic health record systems, and  database systems.
  • Effective verbal and written communication skills to communicate clearly and effectively with patients and others.
  • Very effective oral and written communication skills, including public speaking experience.
  • Effective work organization skills.
  • Ability to work effectively independently and as a team player.
  • Ability to be flexible and work in a changing environment.
  • Sensitivity to the different cultures represented among members and staff.
  • Demonstrated ability to listen and communicate with others in a professional and caring manner, including sensitivity with individuals from diverse cultures and lifestyles.
  • Demonstrated ability to set priorities for tasks to work effectively despite interruptions and under minimal supervision. Self-starter, reliable.
  • Demonstrated proficiency with the electronic health record database within three months of attending training session(s).
  • Fluency in English (speaks, read, and write).
  • Fluency in Spanish (speaks, interpret, read, and write).
  • Ability to solve problems, make routine recommendations, and prioritize.
  • Ability to maintain complete confidentiality regarding health care and other patient or client information.
  • Current California driver’s license, appropriate insurance coverage, and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
