About The Position

The LVN Care Coordinator supports providers and clinical teams by coordinating patient care activities and managing clinical administrative workflows that support continuity of care. This role assists with referral coordination, medication and diagnostic prior authorizations, patient follow-up, and management of clinical communications within the electronic medical record (EMR). The LVN Care Coordinator works collaboratively with providers and clinic staff to ensure timely access to care, efficient coordination of services, and completion of ordered treatments and preventative services. Requirements

  • Knowledge of ambulatory care workflows and care coordination practices.
  • Understanding of insurance authorization processes including medications, imaging, and specialty referrals.
  • Familiarity with EMR systems and clinical documentation processes.
  • Ability to manage multiple tasks and prioritize work effectively.
  • Strong communication and interpersonal skills when interacting with patients, providers, and external healthcare partners.
  • Ability to work independently while maintaining collaboration with clinical teams.
  • Knowledge of patient confidentiality requirements and HIPAA regulations.
  • Strong organizational skills and attention to detail.
  • Ability to provide patient-centered communication and education.
  • Proficiency in computer systems and office software required for clinical care coordination work.
  • High School Diploma or equivalent required.
  • Graduate of an accredited Vocational Nursing program.
  • Current California Licensed Vocational Nurse (LVN) license in good standing required.
  • Current Basic Life Support (BLS) certification from the American Heart Association or equivalent required.

Nice To Haves

  • Some college preferred.
  • Minimum 1-2 years of experience in an ambulatory care clinic or care coordination setting preferred.
  • Experience with prior authorizations, referral coordination, patient follow-up, and EMR documentation strongly preferred.

Responsibilities

  • Coordinate specialty referrals, diagnostic testing, and follow-up services ordered by providers.
  • Assist with medication prior authorizations and communicate with pharmacies, insurance plans, and providers to facilitate medication access.
  • Process and track authorization requests for imaging, specialty care, and other services requiring payer approval.
  • Manage and triage non-urgent clinical messages within the EMR and patient portal under provider direction.
  • Conduct patient outreach for follow-up care, referral scheduling, and completion of ordered tests or procedures.
  • Track referral completion and coordinate communication between patients, specialist, and clinic providers.
  • Review incoming clinical information, laboratory results, and reports within EMR and route appropriately according to established protocols.
  • Assist in identifying gaps in preventative or chronic care services and support patient outreach efforts.
  • Maintain accurate and timely documentation in the EMR.
  • Support quality improvement initiatives and operational workflows that enhance care coordination and patient access to services.
  • Collaborate with clinic staff and providers to ensure continuity of care across services.
