Under the supervision of the Director of Nursing, this LVN position is responsible for supporting adult & child/youth behavioral health needs in multiple programs, including primary care, substance use disorder treatment (Outpatient Based Opioid Treatment and Ambulatory Detox), and outpatient behavioral health programs in assigned work locations. This nurse will support psychiatric providers, primary care physicians, and NPs to ensure appropriate clinic flow and patient appointments are on time. This position will work in collaboration with medical providers and the nursing team to assist with medication refills, health-related education to assist consumers with maintaining stability in the community, medication administration, phlebotomy, and lab testing as ordered by the providers. Other clinical nursing duties may include genetic testing, vital signs, urine drug screening, pregnancy testing as ordered by the Providers. This LVN position performs responsible professional nursing work or professional administrative nursing work in the assigned locations. This position is responsible for the care and treatment of individuals in accordance with accepted professional, legal and ethical nursing standards. Health/medication related services Effectively provide healthcare and medication training and education for multiple programs at various times. Effectively and efficiently process prior authorization request by retail pharmacy and medication insurance providers. Effectively handle inquiries from the onsite pharmacy about medication prescriptions submitted and within eligibility requirements. Medical Crisis Intervention Respond to medical related concerns from individuals receiving services in the various program settings. Make appropriate assessments, collect data, and escalate to higher level of service as needed. Be responsive to program needs--effectively triaging telephone inquiries from individuals receiving services or their family members. Review laboratory results daily. Notify prescribers immediately with abnormal results for consideration for further orders. Accurate, thorough, and timely documentation required. Demonstrates effective problem-solving skills. Teamwork/Cooperation Encourages collaboration, healthy boundaries, and positive relationships within the team. Staff will interact with peers in a positive and encouraging manner. Shares ideas and techniques as well as clinical expertise with team members. Willing to explain and teach information to fellow team members. Sets a positive example of appropriate behavior for other team members, interns, and students. Long- Acting Injectable Antipsychotics Management Prior Authorization for Medication Request submitted to individual’s insurer within 3 business days when injections have been prescribed. (When required by the insurer) Patient Assistance Program applications signed by individual and submitted to the Patient Assistance Program specialist within 3 business days. (If medication is not covered by a different pay source) Schedule and administer LAI as ordered by prescriber in both the Living Room setting and coverage in BH clinics as needed. Staff all cases with the prescriber when necessary. Accurate, thorough, and timely documentation required.
