About The Position

Stanford Health Care – Palo Alto, CA Are you an experienced Advanced Practice Provider (APP) seeking a dynamic and rewarding role in Lung Transplant Medicine? Stanford Health Care’s Lung Medicine Transplant team is looking for a collaborative, detail-oriented NP or PA to join our multidisciplinary program caring for complex pre- and post-transplant patients across inpatient and outpatient settings. Requirements

  • Current or eligible California NP or PA license
  • Demonstrated interest in lung transplant, critical care, or complex medical management
  • Skilled in managing critically ill, high-acuity patients
  • Strong interpersonal skills, teamwork, and adaptability in a fast-paced environment
  • Commitment to lifelong learning and long-term role retention
  • Bachelor’s Degree in Nursing from an accredited college or university Note: PAs hired prior to July 1, 2012 are not subject to this educational requirement
  • Graduate of a PA program accredited by the Accreditation Review Commission on Education for the Physician Assistant.
  • PA - Physician Assistant State Licensure
  • BLS - Basic Life Support
  • DEA - Drug Enforcement Administration

Responsibilities

  • Manage postoperative lung transplant patients across the ICU, step down units and outpatient clinic.
  • Serve in core rotating roles: Inpatient Care
  • ICU/Consult APP: Provide oversight of Lung Transplant patients in critical care as a consulting service
  • Provide floor-based care for lung transplant patients, including daily rounding, admissions, and discharges. This includes management of lung transplant patients from their index hospitalization as a consulting service and following them as a primary service thereafter.
  • Collaborate closely with a robust, attending-driven team including Social Work, Case Management, Pharmacy, Nutrition, and Critical Care.
  • Outpatient Care
  • Conduct shared post-transplant follow-up visits alongside attendings and fellows.
  • Support continuity of care for both recent and long-term lung transplant recipients.
  • Collaborate with multidisciplinary critical care and surgical teams, including fellows, APPs, and other transplant and ancillary services
  • Lead and participate in daily patient rounds, assessments, care coordination, admission and discharge planning
  • Evaluates and treats patients with acute, chronic complaints and health maintenance concerns related to specialty.
  • Obtains complete histories and performs pertinent physical exams with assessment of normal and abnormal findings on new and return patients.
  • Performs or requests and evaluates diagnostic studies as indicated upon evaluation of the patient.
  • Administers medications according to the formulary protocol or upon patient-specific approval from the supervising physician, or transmits orally or in writing on a patient’s record, a prescription from the supervising physician to a person who may furnish such medication. All controlled substances in Schedules II-V inclusive require a patient-specific approval by a supervising physician in advance of transmittal of the order or administration of the medication [see Standardized Procedure: Administering, Ordering, Furnishing or Prescribing of Drugs; Formulary Protocol].
  • Orders and collects specimens for routine laboratory tests, screening procedures and therapeutic procedures, including blood and blood products.
  • Orders physical therapy, occupational therapy, respiratory therapy, radiology examinations and nursing services.
  • Performs designated procedures after demonstrated competency, according to written standardized procedures where applicable.
  • Obtains informed consent, as indicated.
  • Initiates arrangements for hospital admissions and discharges and completes appropriate paperwork.
  • As directed by the supervising physician, enrolls patients in investigational studies approved by the Investigational Review Board (IRB), and orders the necessary tests and medications. [see Standardized Procedure: Administering, Ordering, Furnishing or Prescribing of Drugs; Formulary Protocol] Medications that are not FDA-approved or are used for a non-FDA- approved indication (off-label use) require a patient-specific order in advance from the supervising physician.
  • After appropriate training, assists the supervising physician in the operating room (OR). Acts as first or second assistant under the supervision of an approved supervising physician. Performs surgical procedures in the personal presence of the supervising physician.
  • Recognizes and considers age-specific needs of patients. Effectively communicates and interacts with patients, families, staff and members of the community from diverse backgrounds.
  • Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary.
  • Facilitates the coordination of inpatient and outpatient care and services as needed. Facilitates collaboration between providers and coordination of community resources.
  • Ensures compliance with legal, regulatory and clinical policies and procedures. Participates in quality improvement initiatives.
  • Provides and coordinates patient teaching and counseling.
