About The Position

Under the direction of the Director of Diagnostic Imaging, the Lung Health Nurse Navigator is responsible for the full continuum of cancer patients from screening to detection, diagnosis, treatment and survivorship follow up. Additionally, the Lung Health Nurse Navigator oversees the development, implementation, and maintenance of all aspects of the Low Dose CT screening, the Lung Nodule Clinic and the Lung Cancer Navigation Programs. The overall goal is to ensure cancer patients are effectively and efficiently navigated through the health care system receiving appropriate care throughout the continuum to optimize support, educational services and to provide the communication link for coordination of care among the multidisciplinary health care teams involved. Responsible for establishing, building and maintaining strategic relationships between our community physicians and their office staff. Responsible for promoting, educating and marketing the Advent Health Wesley Chapel Lung Health Program to employed physician, community physicians and their office staff.

Requirements

  • Bachelor's of Nursing Required .
  • 2 Work Experience Required .
  • Driver's License (DL) Required.
  • Basic Cardiac Life Support (BCLS) Required.
  • Advanced Cardiac Life Support Cert (ACLS) Required.
  • Pediatric Advanced Life Support Cert (PALS) Required
  • Registered Nurse (RN) Required

Responsibilities

  • Educates patients regarding expectations before, during, and after diagnostic and treatment procedures, and develops and revises patient educational materials.
  • Builds relationships with community physician practices/groups to grow disease-specific inpatient and outpatient market share.
  • Engages and enrolls employed primary care physicians in the multidisciplinary lung cancer care program.
  • Promotes the Lung Cancer Care Navigation Pathway and multidisciplinary program to community and outreach programs.
  • Researches and develops tools and strategies to promote the Multidisciplinary Lung Program Pathway to area physicians.
  • Visits physician offices regularly, handles physician/physician office issues promptly, and initiates physician communications as needed.
