About The Position

The LTSS Service Coordinator - RN Clinician is responsible for managing members' cases within the scope of licensure, providing supervision to non-RN clinicians, and developing care plans to optimize member health care across the care continuum. This role involves performing clinical assessments, coordinating care for members with chronic illnesses, and ensuring access to necessary services.

Requirements

• Current, unrestricted RN license in applicable state(s).
• Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or Case Management role.

Responsibilities

• Perform telephonic or face-to-face clinical assessments to identify and evaluate member needs.
• Coordinate care for members with chronic illnesses, co-morbidities, and disabilities.
• Develop, monitor, evaluate, and revise individual care plans in collaboration with members and their support systems.
• Establish short and long-term goals for member care in conjunction with caregivers and healthcare providers.
• Assist in the implementation of care plans by facilitating authorizations and referrals for services.
• Interface with Medical Directors and Inter-Disciplinary Teams to develop treatment plans.
• Supervise and direct the work of non-RN clinicians in coordinating services for members.
