The LTSS Service Coordinator is responsible for conducting service coordination functions for individuals in specialized programs within Hamilton/Bradley Counties. This role involves facilitating the Person Centered Planning process, conducting assessments, and developing comprehensive support plans while managing the physical and behavioral health needs of members. The coordinator serves as a single point of contact for care coordination, ensuring efficient utilization of long-term services and supports, and may also mentor new staff and participate in process improvement initiatives.
