About The Position

The LTSS Service Coordinator - Clinician role at Elevance Health involves working under the supervision of a Registered Nurse (RN) to coordinate care for members with chronic illnesses, disabilities, and other health needs. Requirements

• Requires an LPN/LVN, LSW, LCSW, LMSW, or other applicable license as per state law.
• Minimum of 2 years of experience working with individuals with chronic illnesses or disabilities in a Service Coordinator role or similar.
• Current, unrestricted license in applicable state(s) required.

Nice To Haves

• Proficiency in Microsoft Office (Excel, Word, Outlook) strongly preferred.
• Case management experience strongly preferred.
• Home health experience strongly preferred.
• Medicaid experience strongly preferred.
• Discharge planning experience strongly preferred.
• MA/MS in Health/Nursing preferred.

Responsibilities

• Assist the responsible RN with telephonic or face-to-face assessments for member needs.
• Identify and evaluate members for high-risk complications.
• Obtain clinical data as directed by the responsible RN.
• Assist in identifying members that would benefit from alternative levels of care or waiver programs.
• Provide collected information to the responsible RN for care plan development and monitoring.
• Participate in coordinating care for members with chronic illnesses and disabilities in conjunction with the RN and healthcare team.
