About The Position

As a LTSS Service Care Manager, you will assist in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. This role involves developing personalized service care plans for long-term care members and educating members and their families on available services and benefits. You will evaluate member needs, recommend resources, and facilitate care plans to ensure the best outcomes. The position requires approximately 50% travel to visit members in the field, with the remaining time working remotely. The work schedule is Monday-Friday, 8am - 5pm.

Requirements

• Bachelor's degree.
• 2 - 4 years of related experience or equivalent experience.

Responsibilities

• Develop and coordinate holistic care management activities.
• Assist in developing personalized service care plans for long-term care members.
• Educate members and their families on services and benefits.
• Evaluate member needs and recommend resources for optimal outcomes.
• Coordinate between members, families, and care provider teams.
• Provide resource support for various needs based on service assessments.
• Monitor care plans and member status, providing recommendations as needed.
• Interact with long-term care healthcare providers to ensure member needs are met.
• Document and maintain member information and care management activities.
• Perform home and site visits to assess member needs.
• Facilitate education on procedures, healthcare options, and benefits.
• Provide feedback to leadership on quality of care and service delivery.
• Comply with all policies and standards.
