This is a remote field-based role, and bilingual English/Spanish is highly preferred. Florida residency is required, and candidates must live in West Palm Beach County and be comfortable traveling up to 75% within a 30-mile radius. The position assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. Responsibilities include developing personalized service care plans for long-term care members, educating members and their families on available services, evaluating member needs, and coordinating care with healthcare providers.

Requirements

  • Requires a Bachelor's degree.
  • 2 – 4 years of related experience or equivalent experience.
  • RN or LCSW / LCSW-A preferred.

Nice To Haves

  • Bilingual English/Spanish highly preferred.
  • Case Management experience preferred.

Responsibilities

  • Assists in developing, assessing, and coordinating holistic care management activities.
  • Develops or assists with developing personalized service care plans for long-term care members.
  • Educates members and their families/caregivers on services and benefits available.
  • Evaluates the needs of the member and recommends a plan for the best outcome.
  • Assists with developing ongoing long-term care plans/service plans.
  • Identifies providers, specialists, and community resources needed for long-term care.
  • Coordinates between the member and care provider team to ensure services are accessible.
  • Provides resource support to members and their families for various needs.
  • Monitors care plans and member status, providing recommendations as needed.
  • Interacts with long-term care healthcare providers to ensure member needs are met.
  • Collects, documents, and maintains long-term care member information.
  • Performs home and/or other site visits to assess member needs.
  • Facilitates education to long-term care members and their families on healthcare options.
  • Provides feedback to leadership on opportunities to improve quality of care.
  • Performs other duties as assigned and complies with all policies and standards.
