Candidate must live in/around Big Springs, TX. The position assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. This may include developing personalized service care plans for long-term care members and educating members and their families on available services and benefits. The role involves evaluating member needs, recommending plans for the best outcomes, and coordinating between members and care providers to ensure accessibility of services.

Requirements

  • Bachelor's degree.
  • 2 – 4 years of related experience or equivalent experience.

Responsibilities

  • Develop and coordinate holistic care management activities.
  • Assist in developing personalized service care plans for long-term care members.
  • Educate members and their families/caregivers on services and benefits.
  • Evaluate member needs and available resources.
  • Recommend and facilitate plans for optimal outcomes.
  • Identify providers, specialists, and community resources for long-term care.
  • Coordinate between members and care provider teams.
  • Provide resource support for various member needs.
  • Monitor care plans and member status, providing recommendations as needed.
  • Interact with long-term care healthcare providers to ensure member needs are met.
  • Collect and maintain long-term care member information for compliance.
  • Perform home and/or site visits to assess member needs.
  • Facilitate education on procedures and healthcare benefits.
  • Provide feedback to leadership on quality of care and service delivery.
  • Comply with all policies and standards.
