About The Position

Preference will be given to applicants located in Lake County, Illinois near Gurnee, Illinois. The position assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. It may involve developing personalized service care plans for long-term care members and educating members and their families on available services and benefits.
