About The Position

The LTSS Respiratory Care Coordinator (RCC) plays a key role in ensuring quality of care and outcomes for patients with complex respiratory needs residing in Skilled Nursing Facilities (SNFs). This position leverages clinical expertise in respiratory care to provide strategic oversight, consultation, and quality assurance, while fostering strong collaboration with facility staff, administrative teams, patients, families, and organizational leadership. Unlike direct-care roles, the Respiratory Care Coordinator functions as a consultant and advocate, ensuring that facilities adhere to best practices, quality standards, and regulatory requirements. Responsibilities include conducting site assessments, tracking key performance indicators (KPIs) such as readmission rates and infection rates, developing and monitoring Performance Improvement Plans (PIPs), and conducting satisfaction surveys to evaluate both patient and staff experience. This role requires weekly onsite visits to assigned facilities, along with regular communication and reporting to customers and leadership to ensure continuous improvement and accountability.

Requirements

  • Three (3+) years of clinical expertise in healthcare roles.
  • Demonstrated ability to assess, consult, and provide recommendations without performing hands-on care.
  • Demonstrated ability to analyze data and translate insights into actionable improvement plans.
  • Exceptional communication, relationship management, and conflict resolution skills.
  • Strong organizational, time management, and decision-making abilities with a capacity to manage multiple concurrent priorities.
  • Exceptional communication, interpersonal, and relationship-building skills, with the ability to collaborate across clinical and administrative teams.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and capable of working with electronic health records (EHR) and remote monitoring technologies.
  • Strong organizational skills with the ability to meet deadlines, prioritize tasks, and adapt to changing demands.
  • Knowledge of regulatory frameworks and HIPAA-compliant data handling practices.
  • Professional demeanor with emotional stability to effectively navigate patient, family, and facility interactions.
  • Reliable transportation for frequent travel to participating facilities.
  • Associate’s degree in healthcare administration, respiratory therapy, or related field required.
  • Graduate of an approved American Medical Association (AMA)-accredited school for respiratory care by the Commission on Accreditation for Respiratory Care (CoARC).
  • Current, active, and unencumbered state licensure as a certified respiratory therapist, registered respiratory therapist, or respiratory care practitioner (CRT/RRT/RCP) from the National Board for Respiratory Care (NBRC).
  • Valid and unrestricted driver’s license.
  • Proof of current auto insurance coverage.

Nice To Haves

  • Familiarity with healthcare operations or case management support is highly desirable.
  • Bachelor’s degree in healthcare administration, respiratory therapy, or related field preferred.

Responsibilities

  • Collaborate and communicate effectively with facility staff (clinical and administrative), patients, and families to ensure coordinated care.
  • Serve as the primary point of contact for customers, providing program updates and progress reports.
  • Partner with internal team members to identify facility and customer needs for clinical and quality improvement initiatives.
  • Translate evolving standards of care into actionable best practices for participating facilities.
  • Develop, implement, and update Performance Improvement Plans tailored to each facility’s needs.
  • Educate staff on respiratory care equipment, technology, and clinical processes appropriate for skilled nursing environments.
  • Deliver educational in-services and presentations to facility staff, fostering continuous learning and development.
  • Provide ongoing quality oversight for assigned nursing facilities, ensuring adherence to organizational and regulatory standards.
  • Function strictly as a consultant, this position does not provide direct, hands-on clinical care.
  • Collaborate with facilities to monitor and track monthly KPIs, including:
  • Readmission rates
  • Infection rates
  • Unexpected death rates
  • Ventilator liberations and tracheostomy decannulations
  • Offer consultative clinical recommendations for patient care on a weekly basis.
  • Conduct monthly evaluations on selected patients to monitor progress and care effectiveness.
  • Deliver educational in-services to facility staff on respiratory practices, equipment use, and quality standards.
  • Perform quarterly audits at participating facilities to ensure compliance and quality.
  • Administer quarterly satisfaction surveys with both patients and facility staff, documenting findings and reporting results.
  • Provide clear, accurate, and actionable feedback to facilities, customers, and organizational leadership.
