About The Position

Provides assessment, information and linkage for individuals seeking long term care services and supports JOB RESPONSIBILITIES: Provides assessment, information and linkage for long-term care applicants Conduct in-person assessments and data collection Evaluate assessment data to determine program eligibility, care needs, and service options Make Level of Care determinations for assessed long term care NF applicants seeking Medicaid vendor payment Assist individuals in securing appropriate services and care settings Enroll eligible applicants into the PASSPORT Home Care and Assisted Living Waiver programs Apply clinical judgments and perform practice based upon education, experience and professional practice standards Complete assessments utilizing required IT systems and protocols based on contractual agreements with entities other than and including ODA Other Duties Identify and implement LEAN process engineering activities Participate in the development and implementation of the local performance culture Exemplify the Agency 3Cs corporate culture in all activities and interactions with staff, members, consumers, stakeholders as well as the community at larg Maintain minimum proficiency in all requisite computer applications and softwar Participate in the implementation of strategic business and/ or operational plans, projects, programs and systems as appropriate for the objectives Participate in and contribute towards accomplishment of divisional and Agency dashboard goals Perform other tasks as assigned by supervisor Qualifications Position Qualifications This position requires its occupant to have the following abilities to successfully perform the essential functions of the job: Ability to effectively communicate with the elderly population and general public, both in person and by phone Ability to comprehend written material related to essential job functions Ability to accurately document facts related to essential job functions Ability to operate a computer well enough to perform essential job functions Ability to comprehend, recall, and apply facts related to essential job functions Ability to analyze, evaluate and implement a reasonable course of action based on available information Ability to detect specific odors related to the assessment of a client's health/hygiene and the safety of the client's environment Ability to legally drive a car or make other transportation arrangements Ability to ambulate enough to access private homes/locations which may not be readily accessible

Requirements

  • One year of experience in home health care or geriatrics required
  • Registered Nurse in the State of Ohio required
  • ODA PASSPORT certification required
