About The Position

The position is a full-time role at Geisinger Multi-Specialty Clinic, working from 8am to 4:30pm, Monday to Friday. The role requires a Master's Degree in Social Work and a Pennsylvania Active LSW within 12 months of hire. The individual will serve in an expanded clinical role, collaborating with patients, primary care providers, community agencies, and other health plan departments to provide a model of care that maximizes member health status. Responsibilities include integrating evidence-based clinical guidelines into treatment plans, conducting structured clinical assessments, designing individualized care plans, and implementing clinical interventions based on risk stratification. The role also involves providing chronic disease education, conducting in-home assessments, and maintaining documentation for all case management activities.

Requirements

  • Master's Degree in Social Work.
  • PA Active LSW within 12 months of hire.
  • Minimum of 2 years of clinical experience.
  • Knowledge of medical terminology and diagnostic criteria.
  • Computer literacy and critical thinking skills.

Responsibilities

  • Collaborate with patients, primary care providers, community agencies, and other health plan departments.
  • Integrate evidence-based clinical guidelines into treatment plans.
  • Complete structured clinical assessments for identified patients.
  • Design individualized plans of care with members and foster a team approach.
  • Function as a case manager for patients with behavioral health co-morbidities.
  • Implement clinical interventions based on risk stratification.
  • Contact patients through written correspondence and telephone calls.
  • Provide clinical oversight and guidance to team members.
  • Participate in data collection, health outcomes reporting, and clinical audits.
  • Identify patients at risk for poor outcomes and provide intensive follow-up.
  • Conduct in-home member assessments for high-risk and complex cases.
  • Develop collaborative relationships with community-based agencies.
  • Evaluate practices against standards of practice and improve compliance.
  • Maintain required documentation for all case management activities.
