About The Position

The position is a full-time role at Geisinger Primary Care Kingston, requiring a Master's Degree in Social Work and a PA Active LSW within 12 months of hire. The work schedule is Monday to Friday from 8 am to 4:30 pm, totaling 40 hours per week. The role involves collaborating with patients, primary care providers, community agencies, and other health plan departments to enhance member health status. Requirements

  • Master's Degree in Social Work.
  • PA Active LSW within 12 months of hire.
  • Minimum of 2 years of clinical experience.
  • Knowledge of medical terminology and diagnostic criteria.
  • Computer literacy and critical thinking skills.

Responsibilities

  • Collaborate with patients, primary care providers, community agencies, and other health plan departments.
  • Integrate evidence-based clinical guidelines into treatment plans.
  • Complete structured clinical assessments for identified patients.
  • Design individualized plans of care with members and foster a team approach.
  • Function as a case manager for patients with behavioral health co-morbidities.
  • Implement clinical interventions based on risk stratification.
  • Contact patients through written correspondence and telephone calls.
  • Provide clinical oversight and guidance to team members.
  • Participate in data collection and health outcomes reporting.
  • Identify patients at risk for poor outcomes for intensive follow-up.
  • Provide chronic disease and self-management education.
  • Conduct in-home member assessments for high-risk patients.
  • Develop collaborative relationships with community-based agencies.
  • Evaluate practices against standards of practice and improve compliance.
  • Maintain required documentation for all case management activities.
