About The Position

WeCare Centers at South Hills Care Center is seeking an experienced LPNAC to join our team. In this role, you’ll support our nursing facilities by completing MDS assessments and ensuring accurate documentation for quality resident care and proper reimbursement.

Requirements

• Licensed Practical Nurse (LPN) with MDS experience
• Strong attention to detail and accuracy
• Ability to work independently and remotely

Responsibilities

• Complete MDS assessments (MDS experience required)
• Prepare and transmit reports to Medicare & Medicaid
• Ensure accuracy of medical records and coding
• Conduct care area assessments and care planning
