About The Position

Requirements

  • Degree from an accredited school of nursing or a Bachelor's degree in nursing.
  • 4 - 6 years of related experience.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.

Responsibilities

  • Assess, plan, and implement complex care management activities based on member activities.
  • Develop personalized care plans/service plans for care members.
  • Coordinate and manage care between members, families, and care provider teams.
  • Monitor care plans and member status, revising as necessary to meet goals.
  • Identify problems/barriers for care management and appropriate interventions.
  • Review member data to identify trends and improve quality care.
  • Develop appropriate care plans/service plans based on referrals and assessments.
  • Perform outreach to assess member needs and collaborate with resources.
  • Collaborate with healthcare providers to facilitate member services and treatments.
  • Document and maintain member information and care management activities for compliance.
  • Educate members and their families on disease processes and healthcare options.
  • Partner with leadership to enhance care delivery in a cost-effective manner.
  • Precept clinical new hires and support their onboarding process.
  • Engage with cross-functional teams to monitor readiness for service coordination.
