Location: On-site – Geneva or Waterloo, NY Employment Type: Full-Time Weekly Compensation Up To: $1,540.00 Schedule: Day/Evening Rotation (5 x 8-hour shifts) Weekend & Holiday Requirement: Every Other Weekend (EOW) and Holidays Patient Population: Adult & Geriatric Unit Type: Long-Term Care (LTC) Occupational Exposure: Bloodborne pathogens reasonably anticipated Position Overview We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a full-time opportunity in Geneva or Waterloo, NY . This role is ideal for an LPN who demonstrates strong clinical skills, professionalism, and a commitment to patient-centered care in a long-term care environment. Provide direct and indirect nursing care under the supervision of a Registered Nurse Collaborate with an interdisciplinary healthcare team to ensure safe, effective, and high-quality resident care

Requirements

  • Active New York State LPN License
  • BLS Certification (AHA or RQI required)
  • Minimum 1 year of experience as an LPN
  • Minimum 1 year of experience in a Long-Term Care (LTC) setting
  • Strong clinical judgment and nursing skills
  • Excellent interpersonal and communication abilities
  • Professionalism, reliability, and attention to detail
  • Ability to provide care across the adult and geriatric lifespan
  • Ability to work effectively in a fast-paced healthcare environment
  • Commitment to maintaining current nursing knowledge and competencies

Responsibilities

  • Provide direct and indirect skilled nursing care to assigned residents
  • Collect and assess data related to residents' physical and psychosocial conditions
  • Communicate assessment findings to the RN and interdisciplinary team
  • Contribute to the development and implementation of age-appropriate, culturally competent plans of care
  • Recommend revisions to plans of care based on resident outcomes
  • Collaborate with residents, families, physicians, and ancillary staff to promote optimal outcomes and satisfaction
  • Maintain accurate and timely documentation in resident medical records
  • Promote a safe, healthy environment while considering quality, safety, and cost of care
  • Participate in orientation, mentoring, continuing education, and in-service programs
  • Demonstrate leadership and accountability within the scope of LPN practice
  • Comply with all applicable policies, procedures, and regulatory requirements
