Requirements

• NY State LPN License required
• Minimum of one year Med/surg experience in a hospital or Longterm Care facility required
• Must have a NY State Drivers License and a car
• Good computer skills required

Responsibilities

• Reporting to the Coordinator of Care you will be responsible for seeing patients in their home for revisits based on a skilled need, implement the plan of care and evaluate the treatment.
• You will document the visit electronically and completing documentation within 24-48 hours.
