We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Requirements

  • Active New York LPN License
  • BLS Certification (AHA or RQI only)
  • Minimum 1 year of experience as an LPN
  • Ability to work rotating shifts and holidays

Responsibilities

  • Provide direct and indirect nursing care to adult and geriatric residents under the supervision of an RN
  • Collect and assess data related to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team
  • Contribute to and support individualized, age-appropriate, and culturally sensitive plans of care
  • Monitor resident outcomes and recommend revisions to care plans as needed
  • Collaborate with residents, families, physicians, and interdisciplinary team members to promote optimal outcomes and satisfaction
  • Maintain current clinical knowledge through continuing education and in-service training
  • Assist in mentoring and orienting new staff
  • Demonstrate leadership, professionalism, and adherence to safety and infection control standards
  • Perform duties with awareness of potential exposure to bloodborne pathogens
