About The Position

As a LPN, you’ll play a meaningful role in providing nursing services to individuals residing in our ICFs (Intermediate Care Facility). The LPN reports to a Registered Nurse and is responsible for working within the scope of their license to ensure that the physical health needs of the clients are met and maintained on an individualized basis in accordance with policies and procedures and current federal, state, and local standards, guidelines, and regulations. LPN will coordinate health needs with members of the client's team as well as provide necessary training to team members. LPN will also be responsible for delegating nursing tasks to certified direct service providers.

Requirements

  • Must be at least 18 years of age
  • Graduate of an approved school of practical nursing
  • LPN License - State Board of Nursing
  • Must be able to communicate in English, including both spoken and in writing.
  • One year experience working as a LPN with intellectually or developmentally disabled individuals preferred.
  • Must be detail oriented and have competent literacy and writing, documentation, communications and interpersonal abilities.
  • Must be able to use tools and equipment as required – including pushing/pulling wheelchairs, lifting consumers with hoyer lifts
  • Must be able to travel in and out of apartments in order to perform medication passing.

  • Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with residents, staff and support agencies.
  • Must be able to relate to and work with ill, disabled, emotionally upset and at times hostile people within the programs.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
