About The Position

- The Licensed Practical Nurse (LPN) at LIFESTREAM BEHAVIORAL CENTER is responsible for providing high-quality nursing care to patients in a behavioral health setting during Shift 1. The LPN will work collaboratively with the healthcare team to ensure the delivery of comprehensive care, support patient recovery, and enhance the overall patient experience.

Requirements

  • Completion of an accredited Licensed Practical Nurse (LPN) program.
  • 0 to 3 years of experience working as a Licensed Practical Nurse, preferably in a behavioral health setting.
  • Experience in providing direct patient care and working collaboratively within a multidisciplinary team.
  • Strong understanding of nursing care procedures and protocols, particularly in a behavioral health environment.
  • Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare team members.
  • Ability to perform patient assessments and document findings accurately.
  • Proficiency in using electronic health records (EHR) systems for patient documentation.
  • Demonstrated ability to handle stressful situations with professionalism and empathy.
  • Strong organizational skills and attention to detail to manage multiple tasks efficiently.
  • Ability to work independently as well as part of a team, demonstrating flexibility and adaptability in a dynamic work environment.

Responsibilities

  • Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and protocols.
  • Monitor and document patient vital signs, symptoms, and responses to treatment, ensuring accurate and timely record-keeping.
  • Assist in the development and implementation of individualized patient care plans, collaborating with other healthcare professionals to optimize patient outcomes.
  • Provide patient education and support, addressing any concerns or questions to promote understanding and compliance with treatment plans.
  • Maintain a safe and therapeutic environment for patients, adhering to all safety and infection control protocols.
  • Participate in team meetings and contribute to the continuous improvement of nursing practices and patient care services.
  • Respond promptly to patient needs and emergencies, demonstrating sound clinical judgment and decision-making skills.
  • Support the orientation and training of new staff members and nursing students, fostering a collaborative and educational environment.
