About The Position
$5,000 Sign on Bonus - Work where you live! Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home. Team 1 Covers: Bayville, Bethpage, Brookville, East Norwich, Glen Cove, Glen Head, Glenwood Landing, Greenvale, Hicksville, Jericho, Laurel Hollow, Levittown, Massapequa, Massapequa Park, Mill Neck, Muttontown, Oyster Bay, Plainedge, Plainview, Seacliff, Syosset, & Woodbury. 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Great Healthcare benefits, and education reimbursement of $5,250 annually Additional compensation for visits over your caseload
Requirements
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification
Responsibilities
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion s directed by physician’s plan of treatment.
