Job Description JOB SUMMARY: Performs a variety of professional nursing duties in the care of assisted living clients, complies with state and federal rules and regulations, and follows established policies and procedures of the assisted living agency.

Requirements

  • Graduate of accredited school of Nursing for Licensed Practical Nurse.
  • Must be licensed as a Licensed Practical Nurse within the State of Minnesota, and maintain licensure.
  • Must be familiar with general nursing and geriatric nursing practices.
  • Demonstrates and maintains level of professionalism and code of conduct congruent with the Nurse Practice Act and Catholic Eldercare @ Home’s mission and vision.
  • Must possess good listening, verbal and written communication skills.
  • Documents accurate and concise clinical notes in a timely manner.
  • Must be able to train and teach procedures, policies, etc.
  • May be required to work long hours, weekends and holidays.

Responsibilities

  • Monitors unlicensed staff duties and responsibilities as delegated by the supervising RN.
  • Ensures tasks are accomplished in a timely, quality manner, and reporting problems to RN.
  • Monitors client condition through clinical review of systems and reports changes in condition or problems to RN Care Manager
  • Initiate emergency treatment and report to RN Care Manager.
  • Participate in the completion, review and updating of service agreements and service plans
  • Assist with initiation of health care provider orders. Takes and records orders per agency procedure, notifying RN within 24 hours of order prior to implementation.
  • Ensure accuracy and promptness in transcribing prescriber’s orders. (MD, NP, PA)
  • Carry out proper charting practices and procedures in writing and through the use of the electronic charting and scheduled systems.
  • Consult with RN about client conditions and their treatments.
  • Assist with new admissions, transfers and discharges as requested by RN Supervisor.
  • Assembles and operates equipment used for client care.
  • Participates in the ordering and renewal of medications and treatments

  • Perform other duties as assigned by RN Supervisor such as wellness clinics, foot care clinics, etc.
  • Assist in the monitoring and contributes to the evaluation of job performance of unlicensed staff.
  • Consults with RN regarding unlicensed staff performance problems.
  • Assists in planning, assigning and directing work of unlicensed staff as delegated by RN
  • Mentor and assist unlicensed staff as necessary.
  • Provide support for on-call RN through the provision of information and support as ‘first’ call as requested and assigned.
