The LPN is prepared to function as a healthcare team member by exercising sound nursing judgment based on preparation, knowledge, experience in nursing, and competency. Under the direction and supervision of the Registered Nurse and the physicians, the LPN plans, implements and evaluates care and effectively addresses the individual needs and priorities consistent with the patient's cultural beliefs and values and implements select nursing actions within the LPNs scope of practice.

Requirements

  • Education: Graduate of an accredited School of Practical Nursing and passed the Pennsylvania Board Association for Licensed Practical Nurse.
  • Experience: 1 year experience preferred
  • Minimum Certifications, Registration or License Requirements: Current licensure as a Licensed Practical Nurse in the state of practice; BLS required based on organizational guidelines/polices.

Responsibilities

  • Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson.
  • Integrates Evidence-Based Practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal healthcare.
  • Partners with patients, their family/designated care partner(s) in identifying learning needs and priorities and reinforces education in ways that are useful, understandable, and affirming.
  • Recognizes the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs, and cultural beliefs and values.
  • Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care.
  • Creates an environment of belonging and inclusion, demonstrating skills in problem solving and conflict resolution.
  • Actively participates in hospital- and practice setting-based committees, including safety and quality improvement.
  • Demonstrates willingness to give and receive feedback and integrates appropriate changes into practice.
  • Uses information technology to document/communicate, manage knowledge, mitigate error, and support ethical decision making.
  • Demonstrates a commitment to learning and seeks opportunities for continued professional growth and development and acts in primary preceptor role to appropriate staff members
  • Achieve and maintain compliance with all institutional policies and guidelines related to evidence best practices for patient care.
  • Minimizes risk of harm to patients and others by following established nursing practices and protocols.
  • Contributes to monitoring the of nursing quality outcomes to continuously improve the quality and safety of patient care.
  • An LPN shall communicate if the patient’s condition deteriorates or if there is a significant change in condition, the patient is not responding to therapy, the patient becomes unstable or the patient needs immediate assistance. with a registered nurse, physician, and the patient’s health care team members to seek guidance with patient care.
  • An LPN evaluates and collects data as allowed by the licensed practical nurse scope of practice as defined by the Pennsylvania/New Jersey Nurse Practice Act.
  • Work closely with the registered nurse to assure the implementation of safe, quality care.
