About The Position

As an LPN , you will provide high-quality nursing care that adheres to federal, state, and local standards, ensuring elders’ safety and well-being. You will assess, monitor, and deliver treatments while guiding and supervising nursing assistants. Key responsibilities include: Administering medications and treatments as prescribed by physicians. Conducting medical assessments, monitoring vital signs, and responding to changes in condition. Supervising and guiding nursing assistants, ensuring compliance with standards of care. Assisting with elder admissions, discharges, and transfers. Documenting care in electronic health records (Point Click Care) and maintaining accurate clinical records. Communicating with families, physicians, and interdisciplinary teams to ensure elders’ needs are met. Participating in the development and implementation of care plans that address elders’ physical, emotional, and spiritual needs. Educating elders and their families about medications, treatment plans, and wellness practices.

Requirements

  • Graduate of an accredited school of nursing.
  • LPN: Valid Ohio Licensed Practical Nurse license.
  • RN: Valid Ohio Registered Nurse license.
  • Current CPR certification (American Heart Association preferred).
  • Strong communication and critical thinking skills.
  • Willingness to care for older adults and commitment to the organization’s mission.
  • Ability to read, write, speak, and understand the English language.

Nice To Haves

  • Experience in long-term care or geriatric nursing.
  • Proficiency in Point Click Care and Microsoft Office software.
