Job Details Job Location: Laurens County Hospice House - Clinton - Clinton, SC 29325 Position Type: PRN Travel Percentage: Local Travel Job Shift: Day Essential Functions Delivery of Patient Care Participates in the assessment of the patient’s physical and mental health, including the patient’s reaction to illness and treatment regimens. Records and reports the results of the nursing assessment. Participates in implementing the Plan of Care developed by the registered nurse and/or prescribed by any person authorized by State law, by performing tasks assigned or delegated by and performed under the supervision or under orders or directions of a registered nurse, licensed physician or any other person authorized by State law to provide supervision. Facilitates patient and caregiver independence to the extent possible. Participates in the teaching and counseling of patients as assigned by a registered nurse, physician or other qualified professional authorized by State law. Encourages and allows time for questions when meeting with the patient and family. Reports and records the nursing care rendered, and the patient’s response to that care. Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care. Maintains open lines of communication with supervisor. Keeps supervisor apprised of any complex patient/caregiver issues. Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices. Interdisciplinary Group Attends and participates in the interdisciplinary group conferences [IDG] and special care conferences. Seeking and utilizing the appropriate resources for the referral process, as directed by the RN Participates in identifying specific patient/caregiver needs and making referrals to appropriate team members. Documentation Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards. Professional and Personal Accountability Demonstrates competency in knowledge and skills within scope of practice. Ensures that VIA Health Partners professional reputation is maintained and projected. Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice. Maintains current professional licensure, CPR certification and CEU requirements. Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to unforeseen obstacles. May perform other duties as required.
