About The Position

The Licensed Practical Nurse (LPN) at LIFESTREAM BEHAVIORAL CENTER will provide high-quality nursing care to patients with behavioral health needs. The role involves working collaboratively with a multidisciplinary team to ensure the delivery of comprehensive and compassionate care, while adhering to established protocols and procedures. The LPN will assist in the implementation of patient care plans, monitor patient progress, and contribute to the overall therapeutic environment.

Requirements

  • Completion of an accredited Licensed Practical Nurse (LPN) program.
  • Minimum of 1-2 years of experience in a healthcare setting, preferably in behavioral health.
  • Experience in administering medication and providing patient care in accordance with established protocols.
  • Strong understanding of nursing principles, practices, and procedures.
  • Excellent communication skills to effectively interact with patients, families, and healthcare team members.
  • Ability to assess patient needs and implement appropriate care plans.
  • Proficiency in maintaining accurate and detailed patient records.
  • Capability to work collaboratively in a team-oriented environment.
  • Demonstrated ability to handle stressful situations with professionalism and empathy.
  • Basic computer skills for documentation and communication purposes.

Responsibilities

  • Administer medications and treatments as prescribed by healthcare providers, ensuring accuracy and adherence to safety protocols.
  • Monitor and document patient vital signs, symptoms, and responses to treatment, reporting any significant changes to the healthcare team.
  • Assist in the development and implementation of individualized patient care plans, working closely with registered nurses and other healthcare professionals.
  • Provide direct patient care, including assistance with daily living activities, hygiene, and mobility, while maintaining patient dignity and respect.
  • Educate patients and their families on health management and treatment plans, promoting understanding and adherence to medical advice.
  • Maintain accurate and timely patient records, ensuring confidentiality and compliance with regulatory standards.
  • Participate in team meetings and case conferences to discuss patient progress and collaborate on care strategies.
  • Support a safe and therapeutic environment by adhering to infection control and safety protocols.
  • Engage in continuous professional development to stay current with best practices in behavioral health nursing.
