About The Position

Make a real difference in your community! Tennessee Quality Care is looking for a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in the Shelby County, TN area. In this role, you will provide skilled intermittent nursing visits not shift-based or private duty care. Youll support patients recovering from illness, surgery, or managing chronic conditions by delivering clinical interventions in their homes. As a Home Health LPN, you will conduct focused assessments, carry out the plan of care established by the RN Case Manager, and help patients reach their health goals through education, treatment, and proactive care coordination. This role offers the autonomy of fieldwork with the backing of a highly supportive clinical team. Requirements

  • Current LPN license in the state of Tennessee
  • Minimum of one year clinical experience (home health experience a plus, but not required)
  • Strong communication, critical thinking, and time-management skills
  • Ability to work independently in the field while maintaining strong team communication
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • A passion for patient-centered, compassionate care
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Has three (3) years nursing experience.
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance

Nice To Haves

  • Community/home health or medical/surgical experience is preferred.

Responsibilities

  • Provide skilled nursing care under the direction of an RN Case Manager
  • Perform assessments, vital signs, wound care, medication administration, and patient education
  • Execute individualized care plans and document care accurately and timely
  • Collaborate with the interdisciplinary team to support overall patient goals
  • Promote patient safety and advocate for appropriate resources and services
  • Maintain compliance with all agency policies, state regulations, and best-practice standards
