About The Position

The Licensed Practical/Vocational Nurse is responsi b le for p ro v iding d irect p atient care under the supervision of a R egistered Nurse. Responsi b ilities i n clude following the plan of care, pro v iding treatments, and wor k ing collaborati v ely with the mem b ers of the team to h elp meet positive patient care outcomes. Requirements

  • Graduate of an accredited practical nurse o r vocational nursing p rogram.
  • Currently licensed as an L P N/LVN in t h e State(s) o f planned p ractice .
  • Complies with acce p ted professio n al standards and practice.
  • Demonstrates go o d verbal and w r itten commu n icatio n , and organization skills.
  • Must be a lice n sed d river w ith an a u t o mo b ile t h at is insured in acc o rdance with state a n d/ o r organizati o n requirements and is in go od worki n g or d er.
  • Possesses and maintai n s c u rrent CPR certification.
  • Understands phil o sophy of hospice conce p t.
  • Also understands nee d s of the termi n ally ill.
  • Capable of pr o lon g ed or considerable w a l k ing o r standing.
  • A b le to lift, p o sitio n and/or transfer patients.
  • Able to lift supplies and e q ui p ment.
  • Capable of consi d erable reachi n g, stoo p ing b endin g , kneeling, and/ o r crouching.
  • Possesses visual ac u ity and hearin g to perf o rm required nursin g skills.

Nice To Haves

  • One (1) to two (2) y ears nursi n g experience preferred.
  • Community h ealt h /hospice or medical/sur g ical experience is preferred.

Responsibilities

  • Provides d irect patient care as defi n ed in State N u rse Practice Act, inclu d ing specialized duties related t o end of life care.
  • Implements current n u rsin g practice followi n g a com p rehensive assessment and t h e plan of care.
  • Provides accurate a n d timely documentatio n of p atient ser v ices to reflect t h e plan of care.
  • Assesses and provi d es p atient and famil y /caregiver education and inf o rmation p ertinent t o diagnosis and individ u alized plan of care.
  • Participates in coor d inatio n of hospice ser v ices, ap p ro p riately reporting the identified needs to t h e inter d isci p li n ary g roup and Cli n ical Mana g er. E x amples inclu d e hos p ice aide, OT, PT, SW, ST, Volunteer, or D ietician .
  • Uses an d prepares equi p ment and supplies effectively and efficiently.
  • Provides appr o priate pai n /sym p tom management.
  • E v aluates patient’s res p onse to treatments/medicatio n s.
  • Completes mandat o ry education requirements annuall y to maintain com p etence .
  • Attends required staff meetings and IDG.
  • Participates in p ersonal, pr o fessio n al g rowt h and development. A lso participates in organization's qualit y assessment perf o rmance improvement teams and activities.
  • Perf o rms other d u ties as assigned by the Registered N u rse o r Cli n ical Manager .
  • Reports sig n ifica n t changes in patient condition to the patient’s RN Case Manager.
  • May b e required to work o utside of desig n ated geographical area based on census fl u ct u ations and current staffin g .
  • Participates in o n-call d u ties as defined by t h e on-call policy.
  • Acti v ely partici p ates in quality assessment perf o rmance impr o vement teams a n d activities or ot h er committees as assigned.
  • May b e required to v isit p atients in h o mes that are not clean, or in p oor repair, have poor ventilation , are infested with insects or ro d ents, and secondhand smo k e. Driving conditi o ns may be difficult related to weat h er and/ o r traffic.
  • Mai n tains knowledge o f and adheres to all applica b le laws, r u les , and standards.
