CCP specializes in providing comprehensive Cardiology care and currently has over 30 locations throughout the Philadelphia area. We are currently hiring for a full-time LPN to work at our Center City, Philadelphia, PA office. This role supports CCP’s Hypertension Remote Patient Monitoring (RPM) program by monitoring transmitted blood pressure data, responding to alerts, and coordinating nurse-driven clinical management under provider-directed protocols. The position supports hypertension primarily, with potential cross-coverage for heart failure and other chronic care programs. Please note this role is entitled to a $500 sign on bonus.

Requirements

  • Current and Active State licensure as a Practical Nurse
  • Proficiency with EMR systems and accurate clinical documentation
  • Excellent communication, organization, and problem-solving abilities
  • Ability to work independently while contributing effectively to a multidisciplinary team
  • Ability to multitask

Nice To Haves

  • Experience with chronic disease management preferred
  • Cardiology and/or hypertension experience strongly preferred

Responsibilities

  • Monitor incoming hypertension remote monitoring data and alerts.
  • Identify abnormal blood pressure trends and single-reading alerts.
  • Prioritize and respond to urgent vs non-urgent alerts.
  • Perform patient outreach by phone for elevated or concerning readings.
  • Document all outreach, assessments, and interventions in the EMR.
  • Carry out provider-approved nurse-driven protocols for hypertension management.
  • Order labs when required by protocol and follow up on results.
  • Escalate to the cardiologist or APP when blood pressure does not improve or symptoms are present.
  • Educate patients on: Proper blood pressure measurement technique Medication adherence Lifestyle factors affecting hypertension (diet, sodium, activity, stress)
  • Support ongoing engagement to improve adherence and outcomes.
  • Coordinate follow-up appointments after medication adjustments.
  • Communicate clinical updates to providers via EMR phone notes and summaries.
  • Track patient progress and contribute to program outcome data when requested.
  • Participate in evolving workflows as programs scale.

  • If you are hired, and assuming your employment is in good standing, you will receive the sign-on bonus. The bonus will be paid to you no later than 30 days following your start date. In the event that you voluntarily terminate your employment or the Company terminates your employment for cause within 6 months of your date of hire, you will be required to repay the full amount of your sign-on bonus.
