About The Position

Colorado PACE has an immediate opening for a LPN, full-time hybrid, Home Care and Day Center. The organization, Care Synergy, is a network of mission-driven, nonprofit healthcare organizations dedicated to caring for very ill, frail, and primarily elderly individuals by keeping them at home and out of the hospital while maximizing comfort, dignity, and quality of life. The Care Synergy enterprise includes a group of collaborating nonprofit Affiliates across Colorado’s Front Range that share resources, expertise, and infrastructure to deliver high quality, compassionate care. Collectively, the enterprise generates approximately $100 million in annual revenue and serves thousands of individuals and families each year through hospice, home health, palliative care, CAPABLE, wellness, and PACE programs. Care Synergy affiliated programs include The Denver Hospice, Pathways Hospice, Pikes Peak Hospice and Palliative Care, Colorado Visiting Nurse Association, CAPABLE, and Superior Medical Equipment, along with PACE programs including Colorado PACE and Pathways PACE. Requirements

  • Graduate of an accredited school of practical nursing.
  • One year of experience in hospice, palliative care and/or home health, medical/surgical, public health, oncology, acute care or home health.
  • Current Colorado LPN license.
  • Current CPR certification.
  • Valid Colorado driver’s license and proof of automobile insurance in accordance with state statute.

Responsibilities

  • Provides patient home health care in accordance with applicable scope and standards of practice, keeping within the policies, values and mission of the organization.
  • Participates in Interdisciplinary Team/Group (IDT/IDG) meetings.
  • Collaborates with RN Case Manager, Charge RN or assigned RN in the coordination of care to ensure Plan of Care (POC) is being followed.
  • Attends IDG/IDT Meetings as assigned.
  • Provides continuous monitoring of patient’s physical, emotional and spiritual care needs.
  • Demonstrates appropriate nursing care, including communicating with attending provider for medications and supplies.
  • Communicates patient and family information, status and level of care to RN, manager, provider and other IDT members.
  • Assists with coordination and successful completion of patient transfers.
  • Educates patients, families, caregivers and community regarding home health treatments.
