About The Position

The LPN Wound Care Nurse is responsible for the assessment, treatment, and monitoring of residents with acute and chronic wounds in a long-term care setting. This role works collaboratively with the interdisciplinary team to promote wound healing, prevent complications, and ensure compliance with physician orders and regulatory standards.

Requirements

  • Current, active LPN license in good standing in Ohio
  • Minimum of 1 year of nursing experience in long-term care or skilled nursing
  • Experience providing wound care, including pressure injuries, surgical wounds, and skin tears
  • Knowledge of infection control, skin integrity, and pressure injury prevention standards
  • Strong assessment, organizational, and communication skills

Nice To Haves

  • Wound care certification or formal wound care training (preferred but not required)
  • Familiarity with electronic medical record (EMR) systems
  • Experience participating in surveys or quality improvement initiatives

Responsibilities

  • Perform routine wound assessments, measurements, and documentation in accordance with facility policy and regulatory requirements
  • Provide wound care treatments, including dressing changes, skin care, and infection prevention interventions
  • Monitor wound healing progress and promptly report changes or concerns to nursing leadership and providers
  • Collaborate with physicians, nurse practitioners, dietitians, and therapy teams to support comprehensive wound management
  • Ensure wound care plans are current and aligned with physician orders and resident care plans
  • Educate nursing staff, residents, and families on wound prevention and skin integrity best practices
  • Participate in rounds, care conferences, and quality assurance/performance improvement (QAPI) initiatives
  • Assist with pressure injury prevention programs and skin audits
  • Ensure accurate and timely documentation in the medical record
  • Maintain compliance with state, federal, and CMS regulations related to wound care
