JOB SUMMARY The wound Care LPN is responsible for all assigned nursing care activities to include but not limited to wound care for patients of all ages and other clinical activities associated with providing safe patient care. The LPN is responsible for following all WHC policies and procedures, care protocols and Diversified  Clinical Services Practice Guidelines, as directed by a Registered Nurse, or Physician. The LPN will be responsible for assisting with patient and wound care data conferences, and continuing education is required as determined by the Program Director.

Requirements

  • Nursing degree from and accredited nursing school.
  • Current license in the state of practice.
  • Minimum two years experience as LPN/LVN.
  • Minimum one year experience in acute care setting.
  • Familiarity with PC’s and computerized nursing systems and tools, or demonstrated ability to learn.
  • Basic knowledge of regulatory / accrediting requirements for healthcare organizations.
  • Strong interpersonal skills to interact positively and effectively with patients, visitors, staff, and physicians.
  • Good time management skills.
  • Demonstrated ability in assessment and critical thinking.
  • Ability to complete assignments in a timely manner.
  • Accuracy in reporting and documenting information.
  • Professional accountability.
  • BLS - - to be obtained within 30 days
  • Current licensure by the WV State LPN Board
  • Must be able to operate all equipment that is essential to performing job responsibilities.
  • Must have manual dexterity and ability to perform difficult and fine hand manipulation skills.
  • Must be able to walk and stand continually throughout shift.
  • Must be able to frequently lift, pull, and push patients with assistance.
  • Must have functional senses, including sight speech, and hearing for care delivery and effective communication for quality of care.
  • Must have ability to wear protective clothing as needed.
  • Able to twist, turn and bend repetitively throughout the day.
  • Able to crouch, stoop and reach high places.
  • Able to carry, push, and pull and handle patients in addition to multiple pieces of equipment.
  • Able to visually accommodate with good color vision, depth perception and wide field of vision.
  • Able to talk and communicate well with all professionals.

Nice To Haves

  • Wound care experience preferred.

Responsibilities

  • The LPN must be able to demonstrate the knowledge and skills necessary to provide care to patients served on his/her assigned unit as described in unit based competencies, policies and procedures.
  • In collaboration with other staff members, provides bedside patient care, implementing the interventions identified in the plan of care, as directed by RN.
  • Ensure appropriate patient care documentation.
  • Implement and assure compliance with wound center and hospital policies and procedures.
  • Maintain familiarity with regulatory standards.
  • Report quality of care issues to staff RN or Program Director.
  • Ensure availability of necessary equipment and supplies.
  • Communicates with all team members regarding therapeutic interventions.
  • Communicates with staff and physicians to ensure quality of care.
  • Maintains and demonstrates competency in BLS (Basic Life Support), infections control, safety and all unit required skill review.
  • Participates in own employee feedback session identifying individual strengths and areas of growth.
  • Indentifies limitations in own skill/knowledge and seeks consultation from available resources to improve them.
  • Demonstrates commitment to ongoing professional growth and education.
