Requirements

• Current and valid LPN license in the state of employment.
• Proven experience in a clinical or home health setting is preferred.
• Strong organizational and time-management skills.
• Excellent interpersonal and communication abilities.
• Ability to work independently and manage multiple patients efficiently.
• Proficiency in using electronic health records (EHR) systems.
• Valid driver’s license and access to a reliable vehicle for travel to patient homes.

Responsibilities

• Conduct in-home visits to assess patient health and implement care plans as directed by healthcare providers.
• Monitor vital signs and manage chronic conditions by following prescribed nursing interventions.
• Provide education and guidance to patients and their families on health management and disease prevention.
• Administer medications, injections, and treatments as prescribed.
• Maintain accurate and timely documentation of patient care and observations.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive care.
• Exhibit excellent communication skills to foster a positive relationship with patients, families, and caregivers.
• Adhere to established policies, procedures, and regulatory standards.
• Participate in continuous education and training to stay current with best practices in healthcare.
