Requirements

• Ability to pass a pre-hire background check, pre-employment health assessment, and drug screen.
• Valid driver's license, reliable transportation, valid insurance, and clean driving record.
• 1-2 years of clinical nursing experience is preferred.
• Licensed Practical Nurse.

Responsibilities

• Provide nursing care based on care plans, traveling within the service area in all weather conditions.
• Monitor clients' health, report changes, and document care updates in the electronic medical record (EMR).
• Participate in on-call rotations, including evenings, weekends, and holidays.
• Join nursing case conferences, team meetings, and complete required training.
