Requirements

  • Graduate of an accredited LPN/LVN program
  • 3+ years of clinical experience
  • Current state LPN/LVN license
  • Reliable transportation, valid driver's license, and current auto insurance
  • Hospice experience required
  • Tech Savvy

Responsibilities

  • Identify patients who may benefit from hospice services using established internal guidelines
  • Follow internal processes and documentation standards related to transitions, eligibility, and workflows
  • Collect and evaluate physical, psychosocial, social, and spiritual information relevant to patient and caregiver needs
  • Partner with internal teams to ensure accurate documentation, outreach, and coordination of care
  • Meet directly with patients and caregivers to discuss goals of care, service preferences, and initial evaluation needs
  • Educate patients and families about available services, including hospice, to support informed decision-making
  • Teach hospice philosophy, eligibility basics, and the transition process to home health teams and other internal partners
  • Support the hospice branch by coordinating and assisting with admission processes
