Requirements

Active Ohio RN license (unencumbered)
1+ year clinical experience (home care/critical care preferred)
Strong clinical and independent decision-making skills
BLS/CPR certified + TB clearance
Reliable transportation & valid driver's license
Ability to lift up to 50 lbs and assist with patient mobility
Up-to-date vaccinations (COVID-19, Flu, Hep B)

Responsibilities

Provide trach and ventilator care in patients' homes under RN supervision
Assist in implementing individualized care plans and follow care instructions
Monitor patient condition and promptly report changes to the RN or Director of Nursing
Maintain accurate, timely documentation in the home care setting
Follow agency protocols, safety standards, and HIPAA compliance
Educate patients and families on home trach and ventilator care
Collaborate with the home care team to support optimal patient outcomes
Participate in ongoing training and professional development
Provide skilled trach and ventilator care within RN scope of practice
Follow and implement individualized care plans using clinical judgment
Monitor patient condition and report changes to the Director of Nursing
Maintain accurate, timely documentation
Follow agency protocols, safety standards, and HIPAA compliance
Educate patients and families on trach and ventilator care and safety
Collaborate with interdisciplinary teams to optimize patient outcomes
Participate in quality improvement initiatives and ongoing professional development
