About The Position

The LPN will have the honor of caring about the details of high-risk older adults’ care need as part of a highly motivated group who consistently anticipates and follows through on making sure the patient and family needs are met using an interdisciplinary team approach. Applicants should be ready and willing to solve problems together, improve practices to get things right the first time and build trust with patients and families and other health care providers.

Requirements

• High School Diploma or equivalent
• Vocational/Technical Graduate of an approved nursing program
• Successful completion of competency based orientation program
• Healthcare experience

• LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required
• CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required
• CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required

Responsibilities

• Contributes to the assessment, planning, implementation, and evaluation of patient care under the direction of the RN / Mid-level Provider / Physician.
• Collaborates with other members of the health care team.
