The LPN will have the honor of caring about the details of high risk older adults’ care need as part of a highly motivated group who consistently anticipates and follows through on making sure the patient and family needs are met using an interdisciplinary team approach. Applicants should be ready and willing to solve problems together, improve practices to get things right the first time and build trust with patients and families and other health care providers. This LPN will primarily focus on phone triage with the opportunity to work remote part of the time, upon successful completion of orientation and training. Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.
