About The Position

What You Should Know About the LPN Team Float Role:

This is a DRIVING position and will travel to see patient in both homes and facilities within the greater Dayton area
This is a FT position, day shift 8:00am-4:30pm with an every 6th weekend rotation and 1 holiday per year
We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

Requirements

• LPN's - Registered LPN with the State of Ohio without any board actions.
• One year of previous nurse experience.
• CPR Certified
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage

Nice To Haves

• Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)

Responsibilities

• The LPN Team Float provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager.
