About The Position

A long-term care facility is seeking an experienced Licensed Practical Nurse (LPN / T-LPN) to provide high-quality nursing care to residents during overnight shifts. The ideal candidate will have strong clinical skills, experience in long-term care, and a compassionate, patient-focused approach.

Requirements

• Active LPN or T-LPN license in good standing
• Minimum 1 year of Long-Term Care (LTC) experience required
• Strong medication administration and clinical assessment skills
• Ability to work independently during overnight shifts
• Excellent communication and documentation skills

Responsibilities

• Administer oral, topical, subcutaneous, and intramuscular medications, including insulin
• Monitor and document vital signs, blood glucose levels, and oxygen saturation
• Perform wound care, dressing changes, and pressure injury prevention interventions
• Manage feeding tubes, urinary catheters, ostomies, and tracheostomies in accordance with facility policy
• Observe, assess, and promptly report changes in resident condition to the RN or physician
• Collect and handle specimens (urine, stool, blood within scope of practice)
• Maintain accurate and timely documentation in the medical record
• Ensure compliance with facility policies, state regulations, and patient safety standards
• Promote resident dignity, comfort, and confidentiality at all times
