About The Position
The position involves regular communication with residents and their families regarding Plans of Care, Physical Therapy (PT), Occupational Therapy (OT), and other treatment protocols. The role requires participation in morning meetings to facilitate team communication, answering residents' questions about their care and treatment plans, and monitoring and adjusting residents' statuses based on their changing needs. Responsibilities include organizing and prioritizing daily work, completing necessary documentation, performing utilization review activities, coordinating care with various stakeholders, overseeing admissions and discharge activities, and managing referrals. The position demands effective communication and collaboration with residents, families, staff, and professional colleagues, as well as excellent time management skills in a high-volume environment.
