About The Position

The LPN Supervisor for the Assisted Living Program is responsible for providing leadership and nursing care to residents, ensuring compliance with agency policies and procedures. This role involves overseeing nursing tasks, managing medication, and collaborating with healthcare staff to deliver high-quality care in a long-term care setting.

Requirements

• Graduate of an accredited school of practical nursing with a valid and current New York State License as a licensed practical nurse.
• Minimum of one to two years of experience in community nursing, home care, acute care facility, or adult care facility.
• Supervisory and medication oversight experience.

Responsibilities

• Assist with the preparation, implementation, and evaluation of the nursing care plan.
• Oversee the units and assist with managing Home Health Aides (HHAs) and daily tasks.
• Observe, record, and report residents' symptoms, reactions, and changes in condition.
• Perform selected nursing procedures, including administration of medications and treatments.
• Manage resident medication, including changes, new prescriptions, discontinued medications, and refills.
• Assist with admissions and conduct stand-up meetings with HHAs.
• Conduct daily audits of all new admissions and existing charts.
• Assist with incident reporting and follow-ups.
