About The Position

The LPN Homecare Coordinator will be responsible for coordinating and providing high-quality nursing care to patients in a home or clinical setting. The successful candidate will play a crucial role in managing patient care plans, training or supervising home health aides, and ensuring compliance with medical protocols and regulations.

Requirements

  • New Grads welcome!
  • Current LPN license in Georgia.
  • Minimum of 1-2 years of nursing experience, preferably in home healthcare, facility or related settings.
  • Strong clinical assessment and critical thinking skills.
  • Excellent organizational and time-management skills.
  • Effective communication and interpersonal abilities.
  • Proficient in electronic health record (EHR) systems and Microsoft Office Suite (will train).
  • CPR and First Aid certification.
  • Valid driver's license and reliable transportation to travel to patient homes.

Nice To Haves

  • Experience in care coordination or case management.
  • Knowledge of home care (or facility) regulations and compliance.
  • Certification in a relevant specialty area (e.g., geriatric nursing).

Responsibilities

  • Assess patients’ healthcare needs and develop individualized care plans in collaboration with the healthcare team, including the RN.
  • Coordinate and schedule home visits for nursing staff and home health aides to ensure optimal patient care.
  • Provide direct patient care, ongoing communication with case managers, and performing initial nursing assessments.
  • Monitor patient progress and respond to changes in patient conditions, adjusting care plans as necessary.
  • Educate patients and their families about health conditions, care plans, and self-management strategies.
  • Maintain accurate and detailed patient records and documentation in compliance with HIPAA and company policies.
  • Train, supervise and/or mentor home health aides and nursing staff, providing guidance on care delivery and best practices.
  • Conduct regular quality assurance checks and participate in quality improvement initiatives to enhance patient care standards.
  • Communicate effectively with patients, families, and interdisciplinary team members to ensure continuity of care and address any concerns.
  • Stay current on best practices, regulatory standards, and advancements in home healthcare.
