About The Position

This is a home care opportunity where LPNs work to the top of their license by administering care to ElderONE participants in a variety of settings. The community LPN will participate in a team approach to meet the needs of the participants in their homes, or in transitional housing. This may include home visits for dressing changes and medication management and then being assigned to a transitional housing location. These are locations where there are 3-8 participants residing in one place and are in this setting to avoid hospitalization and skilled nursing facility visits, keeping them in the community until they are ready to return to their own residence. The LPN plays a vital role in the participants' success.

Requirements

• Licensed Practical Nurse (LPN) certification from New York State Education Department (NYSED).
• Basic Life Support (BLS) certification from American Heart Association (AHA).
• Desire and passion to work with elderly participants in a variety of settings.

Nice To Haves

• Experience in home care is preferred but not required.

Responsibilities

• Participate in a team approach to meet the needs of participants in their homes or transitional housing.
• Conduct home visits for dressing changes and medication management.
• Support additional ElderONE shared aide facilities and transitional housing areas as needed.
