About The Position

The LPN/RN position at Huntington Creek Recovery Center involves providing and coordinating care by assessing the physical and behavioral health needs of patients. The role includes developing and implementing nursing care plans, maintaining medical records, and educating patients and their families about various health conditions. The position requires adherence to nursing practice standards as defined by state regulations and facility policies. Responsibilities also include ongoing assessment of patient status, collaboration with healthcare teams, and ensuring effective communication with patients and their families.

Requirements

  • Graduation from an accredited school of nursing required.
  • One or more years' nursing experience within the clinical area preferred.
  • Current license as required by state.
  • CPR and de-escalation and restraint certification required (training available upon hire).
  • First aid certification may be required based on state or facility requirements.

Responsibilities

  • Provide and coordinate care by assessing physical and behavioral health needs of patients.
  • Develop and implement nursing care plans.
  • Maintain medical records and educate patients and their families about health conditions.
  • Provide nursing care within the standards and scope of nursing practice.
  • Ensure physician's orders are legible and transcribed appropriately.
  • Assess patient's status on an ongoing basis and document pertinent information.
  • Collaborate with others to meet patients' physical, biopsychosocial, age, developmental, and cultural needs.
  • Notify physician of significant changes in patient status.
  • Ensure nursing treatment plans are specific to assessed needs.
  • Administer medications accurately and document controlled substance administration.
  • Complete admission and discharge processes efficiently.
  • Exhibit therapeutic rapport with patients while maintaining professional boundaries.
  • Provide direction or coordinate unit activities as directed.
  • Communicate therapeutically and professionally with all involved in patient treatment.
