About The Position

The RN or LPN in this hospital-based role serves as a key member of the Wound Care Center team, working in close collaboration with wound care providers and interdisciplinary staff to deliver high-quality, patient-centered care. This team member has developed specialized clinical knowledge, sound judgment, and advanced skills tailored to the unique demands of wound management. Their expertise supports optimal healing outcomes and enhances the overall patient experience.

Requirements

  • Associate Degree Nursing required.
  • Current RN Licensure in State of Iowa or applicable compact state.
  • Must be licensed in state of residence.
  • Basic life Support certification.
  • Valid driver’s license when driving any vehicle for work-related reasons.

Nice To Haves

  • Bachelors of Science in Nursing preferred.
  • Broad-based clinical experience preferred.

Responsibilities

  • Function as a case manager for a defined patient population.
  • Coordinate office visits, pre-procedure work-ups and patient education with the multidisciplinary team.
  • Foster continuity of care between Wound Healing Center and care taker (home, hospital, and nursing facility) across the continuum of care.
  • Assist others within the clinic, triage telephone calls, and assist with scheduling.
  • Initiate a nursing assessment of patient’s condition upon admission, develop a plan of care, develop and implement a teaching plan.
  • Identify patient care situations that require intervention, implement nursing actions and evaluate responses.
  • Identify patient/family education and discharge needs and implement a teaching plan of care.
  • Assess whether the plan of care is effective with the physician and is instrumental in initiating changes as necessary.
  • Implement wound care and hyperbaric oxygen (HBO) protocols when clinically indicated.
  • Accurately complete the super bill and ensure that all orders and charges are initiated accurately.
  • Coordinate activities with other disciplines (rehab, nutrition, diabetic education, home health, durable medical equipment (DME), nursing homes and other ancillary services).
